Hoo's Fit: Health Risk Assessment - Health Screenings
UVA Employees and their spouses who participate in the UVa Health Plan have the opportunity to complete an annual health risk assessment.

At the Health Risk Assessment, you will undergo body composition, lipid/glucose, and blood pressure screenings. You will also complete a questionnaire concerning health and wellness issues.

The questionnaire and screenings are designed to provide feedback that will help you evaluate your current lifestyle, identify health risks, and decide where and how to make improvements. It does not provide a diagnosis of medical problems. A follow-up visit with your primary care physician to confirm the results of the screening and obtain further medical help is highly recommended.

Your personally identifiable information and screening results will be shared only with health improvement programs offered to UVa Health Plan participants including the smoking cessation and disease management programs. You may be contacted by program counselors to assist you in obtaining care if your assessment results point to this as an appropriate action.

A $75 taxable incentive will be paid to the employee in a future paycheck upon completion of the Health Risk Assessment and signature on the Health Risk Assessment consent form by the employee and/or the employee’s spouse.

Health Risk Assessments will be scheduled for the fall of 2009. Watch for an announcement.
