Accidental Death and Dismemberment
Accidental Death and Dismemberment Insurance is provided in addition to Life Insurance. The maximum amount of accidental death and dismemberment insurance at the date of death or dismemberment is determined by the life insurance formula described above. In the case of accidental death, this amount is four times your annual salary, rounded to next $1000 if the amount is not a round number. Accidental death and dismemberment insurance benefits are payable if you sustain bodily injuries while you are actively employed, through violent, external, and accidental means and as a direct result, you lose your life, limb or eyesight within 90 days of the accident.
