Cigna AD&D ORP members are eligible to purchase Cigna Accident Insurance. The Cigna plan provides coverage for death or dismemberment for accidental causes if permanent total disability or death results within one year of the accident. Participants may select benefits, in multiples of $5,000, between the minimum of $25,000 and the maximum of $200,000. The amount may not exceed ten times the participant's annual salary. Faculty members also may select coverage for a spouse and dependent children at a reduced benefit level. Coverage under the Cigna plan terminates on the last day of the month in which active service terminates. Forms: Brochure Group Accident Policy Voluntary Accident Insurance Employee Application Beneficiary Form B. VRS AD&D Accidental Death and Dismemberment Insurance is provided for no additional cost. In the case of accidental death, this amount is four times your annual salary, rounded to next $1000 if the amount is not a round number. Accidental death and dismemberment insurance benefits are payable if you sustain bodily injuries while you are actively employed, through violent, external, and accidental means and as a direct result, you lose your life, limb or eyesight within 90 days of the accident.
